Toltac System® in practice

to use Toltac® System

follow these steps

before surgery

Start with a simple case

As a first step with the Toltac® System, select a simple case (1 or 2 implants in a partially edentulous patient with a dentally supported denture) so that you can focus on how the system works and become familiar with the assistance it provides to the operator.

Test the drivers

Test the drivers with all the drills to be used during surgery, ensuring that each drill engages properly in the handpiece despite the presence of the Toltac® driver. In case of interference, the thickness of the driver's collar can be reduced using a drill to allow for full engagement of the surgical drill's neck in the handpiece. It is preferable to use surgical handpieces without external reinforcement: handpieces with external reinforcement may limit the available height of the drill shank to accommodate the driver and require a reduction in the height of the driver collar, making guidance less precise.

Test the sliding mechanism
driver / guide

Attach a driver to a bur and insert the drill into the handpiece. Practice inserting the tip of the driver into the access port of the guide several times. Slide the elongated part of the driver along the groove of the guide, familiarizing yourself with the friction between the driver and the guide, and learning how to align your movement with the correct direction indicated by the system. Repeat the exercise several times.
PROCEDURE SIMULATION. It can be very helpful to print the model, apply the template, and practice drilling through the model resin with the drills to simulate the procedure.

Test the template

As with any other type of guided surgery, it is important to take the time to ensure that the guide fits perfectly; if possible, schedule a separate session for guide fitting so that any necessary adjustments can be made to ensure the best possible fit, thereby avoiding the need to spend time doing so right before the procedure

Flap or flapless?

Perform the surgical approach according to your preference: flap, flapless, or semiflapless*

Mark
the implant site

This is the most critical part of the procedure. Use a ball-end or lance drill with a high driver (5.50 mm) and never with a low driver, always taking care to keep the drill shank inside the driver to prevent the driver from accidentally slipping off the narrow end of the drill, which would prevent proper guidance. If the play between the Toltac® driver and the Toltac® guides allows the driver to oscillate, mark the implant site at the center of this oscillation. The following drills will align automatically once their tips are inserted into the hole made with the marking drill.

Proceed with the surgery

Perform the surgery using your standard sequence of drills for implant site preparation. Always use the driver with the highest collar compatible with the drills being used, adjusting it if necessary. ALWAYS check the insertion depth of the drills, keeping in mind that the Toltac® System does not provide a depth stop but allows you to see the depth marks on the drills. If available with the system being used, drills or handpieces with bone stops may be used.
